Through the Social Care Network, we will meet with you and your family to talk about what’s going at home and in your life to see if you may need extra help.

If you or your family are eligible, the Care Navigator will help connect you to the support and services you need. Depending on your situation, you may be able to receive more than one type of help.

We will help you with:

• Help to make your home safer and easier to live in, such as installing ramps, handrails, or grab bars.

• Fixing water leaks to help prevent mold and keep your home healthy.

• Sealing holes and cracks to help keep bugs and pests out of your home.

• Providing items like an air conditioner, heater, humidifier, or dehumidifier to help keep the air in your home comfortable and safe.

• Help finding safe, stable housing in your community and support with filling out applications if needed.

• Help from a nutrition expert who can talk with you about your health and help you choose foods that are right for you.

• Home delivery of prepared meals, fresh fruits and vegetables, or groceries for up to six (6) months, based on your health needs.

• Help getting basic cooking items—such as pots, pans, a microwave, refrigerator, or utensils,so you can prepare meals at home safely and comfortably.

• Rides to approved social support locations using public or private transportation.

• transportation to:

  • job interviews,

  • parenting classes

  • housing assistance appointments

  • farmers’ markets

  • government offices

• Getting help with finding a job or job training program

• Getting help applying for public benefits

• Getting help managing your finances

• Getting connected to services like childcare, counseling, crisis intervention, health homes program, and more
